I authorize 10th Planet Jiu Jitsu Dubai, to withdraw from the credit card or bank account identified below for payment of all charges arising under membership account. The information I have provided on myself and below is true, current and complete and I agree to maintain and promptly update such information to keep it true, current and complete.

The payment will be automatically withdrawn from your designated credit card or bank account according to my membership starting date, until such time as written notice of at least 30 business days before the next withdrawal is schedule of revoking the authorization is provided to 10PJJ Dubai

For more information on your right to cancel, contact your financial institution or contact us 10th Planet Jiu Jitsu Dubai may assign your authorization, whether directly or indirectly, by operation of law, change of control or otherwise and shall provide written notice to you following such assignment.

Interruption of service, and cancellation or hold of account.

A) Membership can be frozen for a maximum of 15 days twice a year.

Holds or cancellations of services require 15 days notice in person on a membership amendment form.

Use of service is at the discretion of the member.

Unused services do not constitute grounds for credits or refunds.

B) Prepaid memberships cannot be refunded.

C) In the event of pregnancy, memberships must be frozen for safety reasons.

D) All students under the age of 18 must be accompanied by an adult to all classes and has a legal guardian or parent sign this agreement.

E) If by any reason of death or permanent disability, the participant is unable to complete the program, he/she shall be relieved of the obligation of making payment other than for services performed prior to death or onset of disability.
